command because you can get to admiralty and command a starship where as if you are a blue or yellow shirt you can not do any of that

Medical staff can over ride a captain by giveing them an order if they believe their health is at risk.

I expect that I'd actually join on as a member of the sciences department rather than the medical currently. I'm actually a historian at heart but I'd cross-train as a medic at the very least.

nah i would stay in command because admiralty is only command, how many blue or yellow shirt admirals are there

You don't know what branches a lot of the Admirals came from, I expect a lot are Command branch. Of course, McCoy was an Admiral at one point.

in what episode/movie and he would have to go to command because you have to be a red shirt to get 4 pips

Actually it might be like Dr Crusher at one point - command of a starbase medical facility or the like. He was introduced as Admiral McCoy in "Encounter at Farpoint", admiral of Starfleet Medical.
(And again Arkroyal dives into Memory Alpha!) Reaching captaincy (and thus admirality) is considered more difficult in the non-command branches of Starfleet ("You don't make captain by wearing a gold shirt" DS9: "The Adversary") Captain Krasnovsky - the guy who court martialed Kirk - wore a blue dress uniform indicating that he was science division, Janeway was a science officer before being given a command and Spock was a Captain at one point too and Scotty also.
In alternate/parallel/future universes Beverley Crusher was the captain of a medical ship and Spock, Trip and T'Pol all take over starships at one point or another (though more through chain of command than direct appointment).
Apparently in the US Navy medical captains are senior doctors in charge of Naval hospitals.
